Hydrogen Peroxide
- The principle of hydrogen peroxide for wound debridement:
- Oxidative bactericidal effect :
-When H ₂ O ₂ decomposes, it releases oxygen (O ₂) and hydroxyl radicals (· OH), which can damage bacterial cell membranes, proteins, and DNA, and have a killing effect on anaerobic bacteria (such as tetanus bacilli) and some aerobic bacteria.
-Limitations: The bactericidal effect is short-lived and non selective, and may also damage healthy tissue cells. - Mechanical debridement (foaming effect) :
-H ₂ O ₂ rapidly decomposes into oxygen bubbles upon contact with the wound, and removes necrotic tissue, blood scabs, and foreign objects through physical flushing. This process can assist in removing surface pollutants, but has limited effect on deep necrotic tissue. - Hemostatic effect:
-Low concentrations of H ₂ O ₂ can slightly constrict blood vessels and reduce superficial wound bleeding.

Application process of hydrogen peroxide for wound debridement:
Applicable scenarios:
Superficial wounds: such as small abrasions, cutting injuries, with obvious pollutants (mud, debris) on the surface.
-Wounds with high risk of infection: Preliminary treatment for suspected anaerobic bacterial infections (such as deep stabbing or animal bites).
-Removal of old blood scabs: Softening and loosening of blood scabs or purulent secretions with strong adhesion.
Operation steps:
Concentration selection :
-Medical 3% hydrogen peroxide solution: used directly and cannot be diluted (the bactericidal effect is significantly reduced after dilution).
-Taboo : Avoid using industrial grade high concentrations (>6%) of H ₂ O ₂, which can cause chemical burns.
Cleanup methods:
-Rinse method: Pour H ₂ O ₂ into a sterile syringe or irrigator, gently rinse the surface of the wound, and use foaming to remove contaminants.
-Wet compress method: For necrotic tissue that adheres tightly, cover the wound with gauze soaked in H ₂ O ₂ for 1-2 minutes, then rinse with physiological saline.
Subsequent processing:
-Residual H ₂ O ₂ must be thoroughly rinsed with physiological saline or clean water to avoid continuous damage to newly formed tissues.
-After debridement, apply antibiotic ointment (such as mupirocin) and cover with sterile dressing.
Usage frequency:
Acute phase : limited to one initial debridement use, repeated use may delay healing.
-Chronic wounds (such as ulcers): Routine use is not recommended as it may worsen tissue damage.
