MAGNESIUM SULPHATE

Magnesium sulphate has been the subject of clinical studies with a view to establishing its efficacy as an anticonvulsant. It is a commonly used medication in the treatment of pregnancy-induced hypertension. It is used to reduce blood pressure, treat pre-eclampsia and eclampsia, and is also employed in the treatment of preterm labour.
 

[Pharmacological Actions]

1 Pharmacodynamics Magnesium ions can inhibit the activity of the central nervous system, inhibit the release of acetylcholine from the motor nerve-muscle junction, block the conduction at the neuromuscular junction, reduce or lift the muscle contraction, and at the same time have a diastolic effect on the vascular smooth muscle, dilate the spastic peripheral vasculature, reduce blood pressure, and thus have a preventive and therapeutic effect on eclampsia, and have an inhibitory effect on the uterine contraction of the smooth muscle, which can be used for the treatment of preterm labour.

2 Pharmacokinetics It takes effect 20 minutes after intramuscular injection and almost immediately after intravenous injection. The effect lasts for 30 minutes, and the effective blood magnesium concentration is 2~3.5mmol/L for the treatment of pre-eclampsia and eclampsia, and 2.1~2.9mmol/L for the treatment of preterm labour, with a wide individual variation. Intramuscularly and intravenously, the drug is excreted by the kidneys, and the rate of excretion is related to the blood magnesium concentration and glomerular filtration rate.

Drug interactions] Drugs that are contraindicated with magnesium sulfate include polymyxin B sulfate, streptomycin sulfate, calcium gluconate, dobutamine hydrochloride, procaine hydrochloride, tetracycline, penicillin, and nafcillin (ethoxylated nafcillin).
